Don't Miss Out on Your 2027 Health Incentive

There’s still time to earn your points. To earn 100% of your 2027 health incentive, you must reach Level 4 by Nov. 13, 2026. Your health incentive will be earned on your participation only – spouse participation is not required.

Take advantage of these high point-earning opportunities in your Personify Health account: 

• Complete the Health Check (health assessment) – confidential snapshot of your overall well-being and opportunities to maximize your health. Go to the Health section. 
• Journeys- Reach your goals by taking one step at a time. These self-guided courses help you build healthy habits. Go to the Health section. 
• My Care Checklist- Your checklist gives you rewards for staying on top of your care. Go to the Health section.

How to check your progress 

• App: Click on Rewards. You will see your point total and level completion. Click on Learn How to Earn More Points for details on ways to earn more points. 
• Website: Look at the navy progress bar at the top of the main page. It will show the point total and what percentage of your health incentive that you have earned. Click on   Rewards for details on ways to earn more points. 
• If you need help logging into your account, call Member Services at 844-930-1238.

Points accumulated in 2026 will determine whether participants are eligible to receive a discretionary health incentive in 2027. Your employer will determine in 2027 whether the health incentives will be made for the points earned in the 2026 program year (Nov. 15, 2025 – Nov. 13, 2026).Caregivers earning more than $200,000 are not eligible for the health incentive payable in 2027.Caregivers covered under a collective bargaining agreement receive benefits in accordance with the terms oftheir contract.
