Choose Well: Don’t miss out on your 2026 health incentive

There’s still time to earn points toward your 2026 health incentive. You must reach Level 4 by Nov. 14, 2025 to earn 100% of your incentive.

Take advantage of these high point-earning opportunities in your Personify Health account:

• Complete the health check (health assessment) – a confidential snapshot of your overall well-being and opportunities to maximize your health. Go to the Health section.
• Reach your goals one step at a time through journeys. These self-guided courses help you build healthy habits. Go to the Health section.
• Complete the My Care Checklist, which rewards you for staying on top of your care. Go to the Health section.
• Take the suicide prevention training. Go to the Benefits Section.

How to check your progress

• App: Click on Rewards. You will see your point total and level completion. Click on Learn How to Earn More Points for details on ways to earn more points.
• Website: Look at the purple progress bar at the top of the main page. It will show the point total and what percentage of your health incentive that you have earned. Click on Rewards for details on ways to earn more points.
